Instruments for lamellar keratoplasty
For manual, deep stromal dissection, a spatula set was developed, consisting of three spatulas, colorcoded like the dutch flag. A short, triangled spatula (RED) is used to dissect downward into the cornea, until the desired stromal depth is reached. A half-length, curved spatula (WHITE) with a rounded tip is used to make the dissection up to two thirds across the cornea, and a third, full-length spatula (BLUE) is used to complete the dissection across the cornea over 360 degrees. All spatulas have sharpened edges to create a smooth interface, so that the risk of interface haze development is minimized. Since obtaining a smooth interface may be a crucial step in lamellar keratoplasty, the spatulas may be used up to three surgeries.
An 8-bites, double running 10-0 nylon suture provides an equal distribution of the suture tension over the donor tissue. Placement of the sutures is facilitated by marking the suture entry / exit points on the cornea. After the donor button is positioned in the recipient bed, both donor and recipient tissues can be marked at the same time with the Alternative Suture Entry-point Marker.
Preparation of a donor cornea is greatly facilitated by stable positioning of the donor eye in a Eye Holder with suction fixation. The Eye Holder also proved very effective for wetlab training purposes.
For excision of the recipient posterior lamellar disc, a PLK scissors set was developed consisting of three scissors, colorcoded like the dutch flag. The first pairs of scissors (RED) is used to excise the disc toward 10 and 2 o’clock, the second scissors (WHITE) for excision toward 8 and 4 o’clock, and the third pair (BLUE) for excision toward 6 o’clock.
Vital dyes for anterior and posterior segment surgery
Trypan blue is effective in staining the anterior lens capsule in mature cataract surgery.
Trypan blue is effective in staining epiretinal membranes, and to a lesser extend the internal limiting membrane and the posterior hyaloid.
BlueVisc is effective in visualizing visco elastic during phakic intraocular lens implantation, cataract surgery and other types of ocular surgery.
